What keto is actually being tested against

The most defensible argument for keto and blood pressure runs through the body’s broader metabolic response. Weight loss alone can lower blood pressure, and keto often makes that easier by reducing appetite and lowering total calorie intake without a lot of conscious restriction. The same diet pattern can also improve insulin sensitivity, reduce insulin levels, alter sodium and water handling, and improve glycemic control, all of which can shift blood pressure in the right direction for some people.

Why the literature is still mixed

Recent work compares keto with other structured diets already used for blood-pressure management. The Keto-Salt Pilot Study, published May 20, 2025, compared a low-calorie, high-protein ketogenic diet with a low-calorie, low-sodium, high-potassium Mediterranean diet in overweight patients and patients with obesity who had high-normal blood pressure or grade I hypertension.

In the Keto-Salt Pilot Study, both ketogenic and Mediterranean diets helped lower blood pressure and support weight loss in adults with elevated cardiovascular risk. Keto may be one useful tool, but it is competing with other plans that also improve weight and cardiometabolic markers, especially when sodium and potassium are deliberately managed.

Where clinicians stay cautious

A ketogenic diet can cause adverse lipid changes, electrolyte shifts, and problems if it is pushed too aggressively. That matters more if you are already on blood-pressure medication, because even a modest drop in blood pressure can make a dose feel too strong once diet, weight, and fluid balance start changing.

If someone has obesity, insulin resistance, metabolic syndrome, or another cardiometabolic issue, keto may be interesting enough to discuss under medical supervision.
